Creating online classroom for young electrophysiologists
Hello, everyone. First of all, thank you for your precious time in the "post epidemic" period to participate in the academic link of the 23rd National Forum on interventional cardiology (ccif2020) "young electrophysiologist clinical and basic research excellent paper exchange session". Affected by the epidemic, this year it was changed to an online meeting. In fact, as early as two years ago, the 21st National Forum on intervention cardiology (ccif2018, Yinchuan) encountered a large-scale flight suspension. The meeting was temporarily changed to an online lecture, which achieved good results and accumulated experience for the successful implementation of this meeting. This year's academic session (ccif2020) was jointly organized by Professor Wu Lin, Department of Cardiology, the first hospital of Peking University, and Professor Wang Daowu, Department of Cardiology, Jiangsu Provincial People's hospital.
Ccif2020 "young electrophysiologist clinical and basic research excellent paper exchange special session" is a part of a series of academic activities organized after the establishment of genetic arrhythmia Working Committee of ECG branch of China Medical Biotechnology Association. During this period, we will focus on genetic arrhythmia and present the content of relevant electrophysiological knowledge to you. On the one hand, domestic well-known experts in the field of hereditary arrhythmia will be invited to make a special academic report; on the other hand, some young electrophysiological experts with relatively insightful views in China will be invited to discuss some specific clinical problems / cases. Online conference has its advantages and differences, for example, participants do not need to travel, more people can participate in the membership and exchange, etc., hoping that this academic link can bring you a fresh feeling.
First cardiologist, then cardiophysiologist
I have had the honor to do basic and clinical research in the United States for 12 years and 8 months, and also have done some clinical practice work in the U.S. catheterization room, but most of the time I still do basic research on electrocardio physiology. Generally speaking, it is difficult to learn ECG physiology and the learning curve is long. In the United States, if you want to be an electrophysiologist, you need to do follow-up training for the society of Cardiology after three years of resident training. After graduation from this training, if you want to be an ordinary cardiologist, you can do it for about 5-6 years. However, if you want to be a cardiophysiologist, you need to train for another 2-3 years. Through the cardiophysiologist training, you will no longer be an ordinary cardiologist, but an electrophysiologist. Electrophysiology includes two parts: basic research and clinical research, both of which are well developed. The Chinese people are dexterous and have a good job in the clinical research and treatment of electrophysiology. For example, the level of electrophysiological examination and treatment, including the level of intervention and equipment, has developed at a level comparable to that of foreign countries. However, the difference between the basic research of electrophysiology in China and that of foreign countries is quite large, especially in the training of talents related to the basic research of electrophysiology, which needs to be improved We should strengthen the pertinence.
Medical treatment, teaching and research complement each other and promote each other
Peking University has done a good job in the field of electrophysiology. First of all, from the perspective of teaching, the Department of Cardiology of the first hospital of Peking University attaches great importance to the cultivation of electrophysiological knowledge and skills for young people. In the electrophysiological teaching work, many simple and practical teaching methods have been summarized, which have won many teaching awards in China. On the surface, the knowledge of electrophysiology is very boring. However, the knowledge of electrophysiology has its own logic. Once the logic relationship is sorted out, it will become more and more attractive. The second is basic research. The Department of Cardiology in the first hospital of Peking University has done a lot of work in electrophysiology, including from single cell patch clamp to whole animal, as well as isolated heart and so on. For example, in the past few years, lidocaine test has been used to screen which patients with long Q-T wave have better efficacy on class I B antiarrhythmic drugs (including lidocaine and mexiletine), so as to guide clinical practice and make many critically ill patients recover. We are also summing up the experience in this regard, and striving to publish it as soon as possible for the benefit of more patients. Through the above examples, we can see that the combination of clinical research and basic research can bring very good results for patients.
At the same time, we should be aware that electrophysiologists can't just stay in intervention / pacemaker / ICD, or even let patients go home after some units install ICD for patients with possible sudden death. In fact, this is far from enough, because ICD can only reduce the incidence of sudden death and can't reduce the attack of arrhythmia. We need to pay more attention to how to use drugs better to reduce the attack of patients and the discharge of ICD after installing ICD. As we all know, any attack and ICD discharge is an extremely painful experience for patients. We have summarized the past antiarrhythmic drugs and published a new classification of antiarrhythmic drugs together with experts from Oxford University in 2018. Guiding clinical practice through this new classification is a typical attempt to combine basic research and clinical research, which further promotes everyone's understanding of some special types of arrhythmias and drug targets. This achievement was highly recognized by the European Society of Cardiology in 2018, and was rated as one of the top ten electrophysiological progress in the world. So these understandings also help us to do clinical work better, and then take clinical problems for further research and verification, and then popularize the above progress trends, that is to say, go to the spiral rise of "re popularization, re standardization and re improvement". Clinical, scientific research and teaching complement each other, promote each other, and ultimately benefit more patients.
